Department human resource staff can view their employees’ COVID-19 vaccine status and attachments through the COVID-19 Form Attachments page in SF People & Pay.
Just like the SF Reports & Analytics COVID Vaccination Dashboard, the COVID-19 Form Attachments page reflects only the most recent information submitted by the employee or HR staff on behalf of the employee.
Step-by-step instructions below.
Step 1: Go to the SF Employee Gateway https://sfgov.org/sfc/employee-gateway
Step 2: Click the SF Employee Portal tile and log in. If you have issues logging in, click here to review an article to assist you.
Step 3: From the WORK LINKS tab of your SF Employee Portal, click the SF PEOPLE & PAY tile.
Step 4: From your Workforce Administration homepage, click the Job Data tile.
Step 5: From your Job Data Navigation Collection, expand the Personal Information folder and click COVID-19 Form Attachments.
Step 6: Search for your employee on the COVID-19 Form Attachment Search page. Click the drop-down on the Search by field to select your search criteria.
You can also click the Advanced Search link to expose all Search Criteria fields.
You will receive a ‘No matching values were found’ message if there is no existing COVID-19 vaccine status data for your employee. Otherwise, you will be brought to the COVID-19 Form Attachment page.
Step 7: You have arrived at the COVID-19 Form Attachment page.
The top table provides you information about:
- Your employee’s vaccination status
- Vaccinations dates
- Submitter
Click the View button on the bottom table to view the employee’s attachments.
